JEMS.com is the premier organization and portal for the Emergency Services (EMS) community, and is internationally known by EMS workers as such. They already had a classy site, but needed a survey tool integrated in order to (for the first time ever) put their complex and industry standard EMS Census survey online, and reduce their previous paper nightmare.

To quickly develop a survey tool that included the necessary components:

Delivered in 10 days or less
Intelligently react to user input, using case based logic to determine what questions/answers would be available based on input already provided.
Allow for simple but powerful and detailed reporting.
Integrate seamlessly with the existing JEMS.com site.

JEMS provided us with a page layout/color palette template to work from—we did what we do best: application development.
